[philmont] Looking for the Philmont Medical Form

I always thought that until I read just above the signature line on page
4 of the 2008 form that copies were accepted. The form does say you
must use the current year form which I did not receive until mid Dec.

Call Philmont and see if they will send you a form. I don't think that
Philmont puts their form online as they only allow originals and no down
loads or copies.
