Forms

Health and Medical Record
The BSA Health and Medical Record form is to be completed annually by all scouts and parents who will participate in outings.

Permission Slip
Permission slip is required to be signed by a scout's parent or guardian prior to departing on any outing.

Troop Resource Survey
Identifies the skills and experience possessed by parents, as well as willingness and availability to help with outings and advancement activities.
